Directions
Complete both the Registration/Medical Form and Release of Liability Form below. To complete the Registration/Medical Form you will need a 1) digital copy of your passport, a 2) digital passport photo (head shot), and 3) your flight information. Once you have these items you can complete and submit the form. Do not click "submit" before your files have finished uploading. Please submit one of each form for each trip participant.
Why do I need to submit a passport copy and head shot?
In order to reserve peak permits and trekker ID cards (called TIMS cards) the Nepali government requires scanned passport copies and passport photos (head shots). A passport copy is also required to reserve hotels and book domestic flights for international tourists.
We understand that you may not wish to submit these items online. You are welcome to submit these items by email, mail, or any other method you prefer. Just email us with your preference using the contact form on the main menu at the top of this screen.
Release of Liability Form
I understand that by participating in the activities of Nepal Alpine Guides (NAG) I am exposing myself to a variety of hazards and risks, both foreseeable and unforeseeable. I acknowledge that it is impossible to eliminate risk from NAG’s Programs, and that many of the risks involved in international travel and travel in mountainous areas cannot be predicted or controlled.
These inherent risks include, but are not limited to, the dangers of serious personal injury, property damage, death, and the emotional consequences associated with each of these risks (“Injuries and Damages”). I know that Injuries and Damages can occur by natural causes or by activities of other persons, animals, trip members, trip leaders, trip leader assistants, third parties, or equipment failure either as a result of negligence or because of other reasons.
Specific examples of hazards include, but are not limited to: Trip cancellation, financial loss, rock fall, avalanche, slip and fall, altitude sickness, dehydration, dangerous weather, misuse, failure, or loss of equipment, shortage of food and water, the hazards of traveling in politically unstable areas, the dangers of political disturbance and war, use of dangerous transportation, and acts or omissions of NAG.
I understand that risks such as Injury and Damages are a part of adventure travel such as the programs NAG offers, and I appreciate that I must exercise extra care for my own person and for others around me when faced with such hazards. I further recognize that on the programs NAG offers there may not be rescue or medical facilities or expertise necessary to deal with the Injuries and Damages to which I may be exposed.
I hereby confirm my understanding that, in consideration for my acceptance as a participant on this Program, and the services and amenities to be provided by NAG in connection with the Program:

I have read any rules and conditions applicable to the Program made available to me; and I acknowledge my participation is at the discretion of the leader.

I may be dismissed from a NAG program at any time, without compensation or refund, if I am found to be unfit for participation in a program by NAG staff, leaders, or agents. I am financially responsible for my own transportation in the event of a dismissal, and assume all risks associated with my departure from a NAG program.

I am physically fit and know of no medical or health reason why I should not participate in the activities of this Program.

The Program begins and ends at the location where the Program officially commences, with leader and participants in attendance, as designated by NAG. The Program does not include carpooling, flights, transportation, or transit to and from the commencement location, and I am personally responsible for all risks associated with this travel.

If I decide to leave early and do not complete the Program as planned, I assume all risks inherent in my decision to leave and waive all liability against NAG arising from that decision.
This Agreement is intended to be as broad and inclusive as is permitted by law. If any provision or any part of any provision of this Agreement is held to be invalid or legally unenforceable for any reason, the remainder of this Agreement shall not be affected thereby and shall remain valid and fully enforceable.
To the fullest extent allowed by law, I agree to WAIVE, DISCHARGE CLAIMS, AND RELEASE FROM LIABILITY Nepal Alpine Guides (NAG), its owners, employees, agents, and leaders from any and all liability on account of, or in any way resulting from Injuries and Damages, even if caused by negligence of NAG, its owners, employees, agents, and leaders, in any way connected to this Program. I further agree to HOLD HARMLESS Nepal Alpine Guides, its owners, employees, agents, and leaders from any claims, damages, injuries or losses caused by my own negligence while a participant on the Program. I understand and intend that this assumption of risk and release is binding upon my heirs, executors, administrators and assigns, and includes any minors accompanying me on the Program. This release is a binding legal contract. I have read and understand the rules posted by Nepal Alpine Guides and agree to abide by those rules at all times. I understand that this release is a contract. I sign it of my own free will.
